Anticipatory care

If you wish, any health professional involved in your care can register you at the hospice, or you may self-refer by ringing the advice line. This will enable the service to hold specific information about you, which may be of use to the nurse taking the call. This information can also be used by out-of-hours doctors and district nurses who may need to visit you.

Confidentiality

All patient information will be kept secure and confidential. If needed however the information may be shared with other professionals involved in your care.
